HVA Care Connect

Care Connect is a service available through our partnership with HVA Senior Living Alliance, to Cribbs Residential Center and Thoburn Village residents. With Care Connect, a Certified Registered Nurse Practitioner (CRNP) visits with residents in the safety of their own residence or virtually by way of telehealth. This service provides unparalleled convenience and safety. Additionally, your care is coordinated with your Primary Care Physician, and there are no additional costs or fees.

HVA’s Care Connect provides primary and specialty health care to patients, from assessments and diagnosis to treatment and preventative care. Care Connect treatments include check-ups and physicals, routine exams, blood pressure, and weight checks, immunizations, medication management, and support of chronic conditions. Services are billed to your insurance like your regular doctor appointments. All major insurance, including Medicare, Medicaid, and most managed care plans including Highmark and UPMC, are accepted.
