OPTION #1 -Wesbury at Home Concierge Service Program

Monday through Saturday  –  $1,000/month*

Caregiver for 2 hours, 3 times per week includes:

• Light housekeeping and laundry with linen changes.
• Assistance with bathing, dressing, incontinence management.
• Medication reminders.
• Monitoring special diets and meal preparations assistance.
• Assistance with following home exercise programs.
• Provision of companionship, reading, playing games, completing crafts projects and activities.
• Assistance with caring for pets and houseplants. 
• Provision of respite to family and/or caregiver.

Home delivered meal 3 times per week.

• Includes Wesbury’s standard hot meal of the day.
• Additional ala carte menu items are available and will be billed at the current menu rate.
• A custom meal plan is also available and will be billed at an additional cost.

Personal Emergency Response System

• Includes a standard cellular unit.
• Premium mobile units and/or extra pendants are available at an additional fee and can be tailored to fit your needs.

OPTION #2 – Wesbury at Home Memory Support Program

Monday through Sunday  –  $1,000/month*

Caregiver for 4 hours, 2 times per week

• Caregiver is dementia trained to provide one-to-one care in the home.
• Includes educational material for family.
• Dementia support group is available monthly.
• Priority consideration for admission or respite stay at Wesbury’s Grace Health Center and/or Larcom Memory Support Center. **
• Includes light housekeeping/laundry and linen changes.
• Assistance with bathing, dressing and incontinence management.
• Medication reminders.
• Monitoring of special diets and assistance with meal preparation.
• Assistance with following a home exercise program.
• Assistance with caring for pets and houseplants. 

Personal Emergency Response System

• Includes a Premium Mobile Device, including GPS functionality.
